You need to use a MySQL database to store the entried for each image then a PHP script to pull the data back from the database either using a search or list or whatever you want. I'm sure there are people around here who will be willing to help you out with the script writing.
It's a script that I can think how to write and i'm not that good with PHP and it seems fairly simple to do in my opinion. The date, keywords etc. would be stored in a table withing a database. You can thne write a script to enter the data manually with your own keywords or automatically somehow.
I'd help out u i'm sure there are people who have more free time than me at the moment and a clearer view of how it's all done.
