Look next to your name, it will show you your country's flag.

They are most probably IP based yes, ISP based would be the same actually. The other thing they could check for is the accept language the browser sends but that's not as accurate.

No, they're based by IP. There are a couple of companies that keep an extensive and accurate database of IP-to-country records that I had to import for this to work.
